Note to retirees eligible for our Supplemental Health & Welfare Fund:

Supplemental Health & Welfare checks for the period of January - June 2007 were mailed on Monday, January 14, 2008.  Claim forms for the next reimbursement period, which covers premiums paid by retirees for the period of July - December 2007 were also mailed on that day.  PLEASE REVIEW YOUR CLAIM FORM CAREFULLY BEFORE SENDING IT TO THE UNION HALL!  If any of the information listed on your form is wrong, contact Kathy at the union hall immediately.

Claim forms are due back at the union hall no later than June 30, 2008.  Any claim received after that date will not be processed.  Keep in mind that a portion of the spouse's premiums (if applicable) will be reimbursed for the period of July - December 2007. Please note the following:

* If you have insurance through the International, then your spouse's premiums are already detailed on your claim form, and you do not need to provide any more information.

* If your spouse has insurance through a company OTHER than the International, then you will need to provide premium information for the give reimbursement period.
