October 2011 - notice from the Health & Welfare Fund to all retirees with
Medicare primary coverage:
Participants and their dependents who have Medicare and Retiree coverage through
the Fund will begin receiving their enrollment and 2012 benefit information from
UnitedHeathcare within the first couple weeks of November 2011.
Participants will be automatically enrolled unless they choose to "opt out" of
this coverage. If a participant chooses to opt
out of this coverage THEY WILL LOSE COVERAGE THROUGH THE FUND.
It is very important for Participants considering opting out of this coverage to
contact UnitedHealthcare at (877)755-5349 to discuss benefits.

National Funds will change to
8:00am - 4:00pm Central Standard
Time, Monday - Friday.
This includes the Annuity Department, the Pension Department, the Health &
